Hospice RN Case Manager: Care Coordination Leader
Four Seasons
Four Seasons is seeking a Support RN to uphold our hospice mission in the West Region and surrounding areas. This role reports to the Clinical Team Leader and is structured as hourly, non-exempt, with a 40-hour work week and a Monday–Friday 8am–4:30pm shift. The RN will perform admissions and patient visits as needed, conduct comprehensive patient assessments, and deliver direct nursing care aligned with the care plan and regulatory requirements. #J-18808-Ljbffr Four Seasons
