Physical Therapist - Home Health

Key Responsibilities:

• Perform initial evaluations and develop individualized treatment plans to help patients improve mobility and regain independence

• Provide therapeutic exercises, manual therapy, and mobility training to patients in their homes

• Assess patient progress and adjust care plans accordingly

• Educate patients and caregivers on safety and exercises

• Maintain timely and accurate documentation of patient care

• Collaborate with interdisciplinary care teams and adhere to home health and Medicare guidelines

Qualifications:

• Current Physical Therapist license

• Graduate of accredited PT program

• 1-2 years of experience preferred in Home Health

• Strong communication skills

• Ability to work independently

• Possesses and maintains current CPR certification

• Valid driver's license and reliable transportation

• Knowledge of OASIS

• Experience with EMR systems such as Kinnser

About the Salary:

Our competitive salary range for this role is $100,000 - $120,000+ annually, commensurate with experience.
