Job Summary:

Allaire Health Services is seeking a dedicated and compassionate Physical Therapist (PT) to join our team at our Rutland, Vermont, United States facility. Responsibilities:

• Evaluate patients' physical function and develop individualized plans of care in collaboration with the healthcare team

• Implement and conduct physical therapy interventions, including exercises, manual therapy, and other modalities

• Monitor patients' progress, adjust treatment plans as needed, and document patient information accurately

• Educate patients and their families on physical therapy techniques and provide coping strategies to promote independence

• Collaborate with other healthcare professionals to ensure comprehensive care and communication

• Maintain accurate records and participate in quality improvement initiatives
