Job Summary Monitors patients for and provides education on food/drug interactions, records food and drug interactions in the education section, and consults with physicians regarding appropriate diet modification. Responsibilities Screen and assess patients accurately and timely based on levels of care; determine patients' nutritional needs and status and provide follow-up care. Interview patients and/or their families to obtain history of food habits and preferences. Interpret pertinent laboratory, physical, and social data in the medical record as it applies to nutrition care. Provide nutrition education and record patient's diet and diet instructions, ensuring understanding is documented. Modify patient menus as needed. Qualifications Graduate from an accredited college/university with a degree in Dietetics or Foods and Nutrition. Minimum one (1) year of experience. Two (2) years of experience preferred. Master’s degree preferred. For post-acute locations: New York State Certification issued by NYSED as a “Dietitian‑Nutritionist”. For acute locations: Registration by the Commissions on Dietetic Registration (CDR) as a Registered Dietitian Primary Source Verification.
