• Coordinate resident assessments, MDS completion, and care planning processes

• Ensure accurate documentation for reimbursement and regulatory compliance

• Collaborate with interdisciplinary teams to ensure quality resident care

• Develop, review, and revise individualized care plans as needed

• Monitor resident progress and coordinate the continuum of care

• Support regulatory compliance and maintain current knowledge of long-term care practices

• Help ensure residents receive the resources and services needed for optimal outcomes

• Current Illinois RN or LPN license required

• Long-term care experience required

• One to two years of MDS experience preferred

• Knowledgeable in long-term care nursing practices and procedures

• Organized, detail-oriented, and team-focused professional

• Passionate about providing compassionate, resident-centered care
