Responsibilities: Conduct comprehensive patient assessments and develop individualized care plans Administer medications, treatments, and skilled nursing procedures in a home health setting Monitor and document patient progress, vital signs, and changes in condition Coordinate care with physicians, therapists, and other healthcare team members Educate patients and family members on disease management, medications, and home safety Ensure compliance with state, federal, and agency regulations and documentation standards Requirements: Active Registered Nurse (RN) license in the state of Maryland Previous home health, med-surg, or community nursing experience preferred Strong clinical assessment and critical thinking skills Excellent communication and patient education abilities Own transportation and valid driver's license for home visits Proficiency with electronic health records (EHR) and clinical documentation Compassionate, self-motivated, and able to work independently About Us: ComForCare In-Home Care - Baltimore/Carroll is a trusted provider of in-home health and personal care services dedicated to helping clients live their best lives possible.
