Responsibilities:

• Conduct accurate and thorough MDS assessments of residents, identifying their physical, emotional, and social needs

• Develop and implement individualized care plans that address the unique needs of each resident

• Collaborate with other healthcare professionals to deliver high-quality care and achieve optimal outcomes

• Communicate effectively with residents, families, and colleagues to ensure seamless transitions and continuity of care

• Participate in quality improvement initiatives and contribute to the development of policies and procedures
