LPN/LVNTryfacta is seeking a LPN/LVN for our client in Sonora, CA 95370. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!Location: Sonora, CA 95370Duration: 13 weeks (possible extension)Work Schedule: Day shift - 40 hours per week (8x5's) Guaranteed hours: 40Responsibilities for this position include, but are not limited to:Deliver compassionate nursing care to hospice and palliative care patients.Administer medications, treatments, and physician-ordered therapies.Monitor and document patient condition, pain management, and symptom control.Collaborate with physicians, RNs, social workers, and interdisciplinary hospice teams.Provide emotional support and education to patients and their families.Maintain accurate electronic medical records (EMR) and nursing documentation.Assist with care planning and end-of-life comfort measures.Follow infection control, safety, and hospice care protocols.To be considered for this position, you should have:Active California LVN/LPN license.Minimum 1 year of recent LVN/LPN experience (hospice, skilled nursing, home health, or long-term care preferred).Current BLS certification (AHA).Experience with hospice or palliative care is preferred. Strong clinical assessment, communication, and documentation skills.Ability to work independently and as part of an interdisciplinary care team.Willingness to work scheduled weekends.Tryfacta is an Equal Opportunity/Affirmative Action Employer.
