To be directly involved in ensuring the quality of care to all individual(s) served, including freedom of choice, prevention of abuse, neglect, or exploitation. To work closely with the individual, the legally authorized representative (LAR), with family members or advocates in the delivery of services to the individual served. To work with other provider agencies, consultants, the MRA, and with agencies or entities providing generic resources. Will perform only case management services for the HCS Program. Essential Job Functions Coordinate and monitor the development and delivery of HCS Program Services and services from other sources, according to HCS guidelines. Coordinate and ensure that the individual(s) served's plan of care is in accordance with team members’ recommendations and reflects the results of assessments, personal goals, and choices of the individual served and the legally authorized representative or family. Will obtain all necessary consents and maintain them in the appropriate files. Coordinate consultant services in accordance with the IPC and ISP. Ensure that the IPC, the ISP, the LOC, and the LON are current and in the individual’s record and that all individual(s) served meet the HCS guidelines. Coordinate or supervise the coordination and implementation of plans, consultant services and services by vendors. Monitor the delivery of these services. Maintain contact with the individual, family or legal authorized representative, providing phone and pager numbers. Inform them of any changes in the ISPs. Ensure the individual(s) served, family members or the LAR are provided with copies of Individual Rights, Grievance Procedures, phone numbers and information on contacting TDPRS to report abuse, neglect, or exploitation, and pertinent Evergreen Policies and Procedures. Coordinate pre-discharge and discharge plans, as well as temporary discharges. Ensure that Day Program, Supported Employment, Vocational Services, and other services provided to individuals meet the HCS guidelines. Ensure the HCS guidelines are met for pre-school and school age children. Ensure effective utilization of Evergreen or community resources or services. Monitor or oversee the monitoring of all plans Develop and maintain individual(s) served records in accordance with federal, state and HCS guidelines. Provide records and reports to HHSC, TDPRS, CMS or other regulatory agencies and work with them in a spirit of cooperation. Chair the ID Team meetings. Attend meetings for individual(s) served, including school staffing and doctor appointments as needed. Arrange for transportation as needed. Monitor financial records periodically to ensure that the accounts meet Social Security and Medicaid requirements. Ensure room and board agreements are current and appropriate. Enter and monitor CARE data (consumer demographics; IPC entry and monitoring; discharges temporary and permanent; provider locations) Chair and conduct quarterly advisory committee meetings. Ensure monthly invoicing for Host Home services is completed and submitted. Review all service delivery logs (home and day hab) to ensure they are accurate and completed per HHSC requirements. Forward the monthly service delivery logs for billing after review. Ensure monthly OIG checks for Host Home providers are completed. Other duties as assigned by the Executive Director Qualifications/Experience/Job Knowledge Bachelor’s degree with specialization in Psychology, Social Work, Human Services, or a related field preferred Must have three (3) years’ experience in the field of developmental disabilities, with at least one (1) year in a supervisory role. Working knowledge of computers including use of Google Workspace as well as the aptitude to learn additional software as required. Physical Requirements Constantly moves about to coordinate work. Regularly works in fast pace environment with multiple task deadlines. Regularly moves and positions objects weighing up to 50 pounds. Occasionally exposed to viruses and infectious conditions. Constantly alert and observant during working hours. Reasonable accommodations may be made to enable individuals with disabilities or a medical condition to perform the essential functions. Special Requirements May be required to attend seminars or job-related training courses. Must have understanding, patience, and tact in dealing with individuals with intellectual and developmental disabilities. Must have the ability to maintain good working relationships with Evergreen staff and with contacts from other agencies or entities. Able to participate effectively in the planning and negotiation process for the individual’s Plan of Care and Person-directed Plan and have the ability to further develop or assist in the development of Person-directed Plans. Ability to identify individual’s strengths, weaknesses, and developmental capabilities and develop training objectives.

Employment Variables Must have a good driving record which meets the minimum requirements for Evergreen and reliable transportation. Must pass a drug screen and criminal background check. Must be able to work some evenings when required.
