Perform evaluations and ongoing assessments to monitor progress and refine individualized treatment plans. Deliver therapy interventions using evidence-based protocols and adapt techniques to meet patient goals. Collaborate with an interdisciplinary team, and co‑lead patient groups as appropriate. Complete comprehensive clinical documentation within established deadlines and communicate discharge plans to patients, families, and care partners. Supervise assigned PT aides or assistants and ensure therapy quality and compliance with standards. Requirements Hawaii state Physical Therapist license (active) 2 years of physical therapy experience (required) 1 year of physical therapy home health experience (required) Bachelor's degree in Physical Therapy; Master's degree preferred Homecare Home Base and OASIS experience required CPR and BLS (AHA) certifications Travel experience preferred; willingness to travel within the assignment area Ability to work effectively in a home health care setting and manage patient care in a community-based environment Benefits and Support Guaranteed hours: 40 hours per week Mileage reimbursement: $0.43 per mile Assignment support from Healthforce Healthcare, LLC Travel coordination and scheduling assistance #J-18808-Ljbffr
