Location: Santa Rosa, CA Job Type: Full-time / Part-time Pay: $45.00 - $65.00 per hour

Job Summary

We are seeking a motivated and energetic Physical Therapist (DPT/PT) to join our outpatient rehabilitation team. This position offers flexible full-time or part-time scheduling and the opportunity to work with patients of all ages, specializing in orthopedic rehabilitation, sports injuries, and functional recovery. New graduates and experienced clinicians are encouraged to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations to assess patient conditions and functional limitations.
• Develop and implement individualized treatment plans using evidence-based rehabilitation techniques.
• Provide therapeutic exercises, manual therapy, and other interventions to restore mobility and reduce pain.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
• Educate patients and families on injury prevention, home exercise programs, and self-management strategies.
• Maintain accurate and timely documentation in the electronic medical record (EMR) system.
• Collaborate with physicians and multidisciplinary healthcare professionals to ensure coordinated patient care.
• Assist with discharge planning and continuity of care when appropriate.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Current Physical Therapist license in California (or eligibility for licensure).
• Experience in outpatient orthopedics, sports rehabilitation, acute care, or home health preferred.
• Strong knowledge of anatomy, physiology, biomechanics, and rehabilitation principles.
• Familiarity with EMR documentation systems.
• Experience treating orthopedic, sports medicine, pediatric, and geriatric populations is a plus.
• Excellent communication, interpersonal, and clinical reasoning skills.
• Ability to work independently and as part of a collaborative healthcare team.
