Mobile Integrated Care RN Manager - Home Visits

CareSource

CareSource Massachusetts seeks an Integrated Care Clinical Manager (RN) to lead complex care management for dually-eligible enrollees, coordinating health services and linking community resources to improve outcomes and quality of life, with travel across communities to visit members in homes and other settings.

You will supervise the interdisciplinary care team, assess needs, develop care plans, educate members and families about Medicare/Medicaid benefits, and ensure seamless transitions
