Intensive Care Coordinator Supervisor

At CSR, our Behavioral Health Department (BHD) offers a comprehensive array of high quality, culturally-sensitive mental health services that provide children and families with the services they need for success at home, in school, and in the community. Job Summary Intensive Care Coordinator Supervisor supports Intensive Care Coordination (ICC) which is a service that facilitates care planning and coordination of services for MassHealth youth, with serious emotional disturbances, under the age of 21, and enrolled in MassHealth Standard or CommonHealth who meet the medical necessity criteria. Care planning is driven by the needs of the youth and developed through a Wraparound planning process consistent with Systems of Care philosophy. Essential Duties Ensure that all Intensive Care Coordinator (ICC) staff complete all required trainings, including Child/Adolescent Needs and Strengths (CANS) Assessment and CANS certification. Training is completed upon initial employment and annually. Ensure that all ICCs provide care coordination to youth/families. This service includes but is not limited to the following: facilitates the development of care plan teams (CPT); conducts CANS and risk assessments; convenes CPT meetings; coordinates and communicates with the members of the team to ensure the implementation of the individualized care plan; works directly with the youth and family to implement elements of the care plan; coordinates the delivery of available services; monitors and reviews progress toward care plan goals and updates the care plan. Ensure that the ICCs conduct all necessary assessments in a timely manner and that they are reviewed and approved as required by the mandate. Ensure that ICCs maintain paper and electronic records in an orderly and timely manner. Conduct weekly supervision to ICCs and ensure all supervisory notes are maintained and readily available for review by auditors during site visits. Meet weekly for supervision with the Assistant Director; ensure all cases are discussed and that supervisory meetings are documented according to guidelines. Ensure Medical case consultations occur as needed. Ensure ICCs work closely with Family Partners and the Senior Family Partner is kept abreast of active cases. Ensure ICCs follow-through with on-call policies and procedures. Work with Director to ensure the necessary trainings are conducted upon employment and annually. Ensure all fidelity monitoring activities required by EOHHS, payers and CSR are being conducted. Ensure all services are provided in a professional manner, ensuring privacy and safety. Accurately record hours worked and leave time (if applicable) in Electronic Health Record and ADP in accordance with company and department policy Perform other related duties as assigned, within your scope of practice or function area. General Must comply with all regulations Ability to respect and communicate effectively with co-workers and managers Adheres to agency and departmental regulations, policies and procedures Mandatory reporter of actual and suspected abuse and knowledgeable of agency mandatory reporting practices regarding any suspected cases of abuse, mistreatment, or neglect Requirements And Qualifications An Associate’s degree or High School Diploma (equivalency) and a minimum of five (5) years of experience working with the target population. A minimum of 3 years supervisory experience in a similar setting. Must be certified in the Massachusetts Children’s Behavioral Health Initiative CANS. Must be able to meet with assigned families in their home and/or in the community and to transport families as needed. Experience in navigating child/family-serving systems and experience advocating for family members who are involved with behavioral health systems Must have excellent writing and communication skills. Knowledge of referral coordination to community and private/public resources. Available and able to manage own work schedule to meet each family’s scheduling needs during business hours (M-F, 8am – 8pm).
