Physical Therapist (PT) – Home Health

Location: Concord, Tilton & Manchester, NH Territory
Pay: $45.00–$52.00 per hour
Schedule: Full-Time | Monday–Friday | 8:00 AM–5:00 PM
Productivity: 5 patient visits per day

Position Summary

As a Home Health Physical Therapist, you'll provide individualized, one-on-one therapy services to patients in their homes throughout the Concord, Tilton, and Manchester areas. You'll evaluate patients, develop personalized treatment plans, and help individuals regain strength, mobility, independence, and confidence while collaborating with physicians, nurses, and the interdisciplinary care team.

Responsibilities

• Perform comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide therapeutic interventions to improve strength, balance, mobility, and functional independence
• Educate patients and caregivers on exercises, safety, and disease management
• Assess home environments and recommend modifications when appropriate
• Collaborate with physicians, nurses, and other members of the care team
• Complete timely and accurate documentation using KanTime EMR
• Participate in interdisciplinary case conferences and care planning

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
• Current Physical Therapist license in New Hampshire
• Strong communication and interpersonal skills
• Excellent organizational and time management abilities
• Comfortable using electronic medical records (EMR)
• Home health experience is preferred but not required
• We welcome clinicians from outpatient, acute care, skilled nursing, rehabilitation, or other therapy settings who are interested in transitioning into home health
