CASE MANAGER

Job Overview Case Manager at Walton Academy for Growth and Change, DeFuniak Springs, FL. Provides residential treatment to improve the lives of youth through mental health and substance abuse services, and administers individualized case plans and treatment programs as part of the Rite of Passage Program. Responsibilities Develop and facilitate the completion of case plans and treatment programs in accordance with agency requests and student needs. Administer and monitor student progress and treatment program efficacy. Report directly to the Director of Student Services. Qualifications Bachelor’s degree in a related field (Master’s degree preferred). Prior case management experience with juveniles; three years’ experience required. Successful completion of a criminal background check, drug screen, physical exam, and TB test. Excellent verbal and written communication skills and ability to maintain confidentiality. Flexible schedule and ability to work more than 40 hours per week as needed. Ability to handle multiple priorities and work effectively in a team environment. Benefits Pay: $21.92 per hour. $1,000 Training Incentive when the Essential Skills Training Program is completed within the first 90 days of employment.
