Compassionate Hospice RN Case Manager

IntelliResume is looking for a Hospice Registered Nurse Case Manager to provide compassionate care for patients in Cheshire, Connecticut. The role involves coordinating patient care, managing interdisciplinary teams, and educating patients and caregivers on hospice services. The ideal candidate should have a Nursing Diploma or an Associate's Degree in Nursing with at least two years of nursing experience, preferably in home care or hospice.
