RN Care Coordinator - Patient Transitions

Parkland Health is seeking an experienced RN to join the Care Coordination team at New Parkland Hospital. The role focuses on determining patient eligibility, developing individualized care plans, and coordinating with the healthcare team to ensure quality outcomes for patients across the continuum. You will manage utilization review, discharge planning, and communication with patients, families, and payers while upholding Parkland policies.
