- Shift Type: Varied
- Board Certified in Pediatrics - (REQUIRED)
- TRULY Board Eligible (within 5 years of fellowship working toward BC) or Board Certified in Neonatal-Perinatal Medicine - (REQUIRED)
- KS license (or willing to obtain) - (REQUIRED)
- Fellowship-trained in Neonatal-Perinatal Medicine (to be completed prior to start) - (REQUIRED)
- Clean malpractice and license history - (HIGHLY PREFERRED)
- Most recent NPDB Self Query Report (within 30 days active) - (REQUIRED) * no exemption, required at presentation
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - (REQUIRED)
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) (REQUIRED) * no exemptions
- Fellowship-trained in Neonatal-Perinatal Medicine (to be completed prior to start)
- Does the provider require visa support - J1 or H1B?
