Physical Therapist Case Manager Home Franklin Team

CarePartners is seeking a dedicated Physical Therapist Case Manager for our Home Franklin Team within our Healthcare & Medical Services department. In this unique role, you will combine your clinical expertise in physical therapy with case management responsibilities to support CarePartners of Connecticut members receiving care in the home and community. You will coordinate and oversee care plans for patients who require physical therapy and related rehabilitative services, ensuring they receive the right care at the right time from network providers. Working closely with physicians, home health agencies, rehabilitation partners, and internal care management teams, you will assess functional status, review clinical documentation, and use your professional judgment to help design, implement, and monitor individualized care plans that support safe mobility, recovery, and independence. You will communicate regularly with patients, family caregivers, and providers to clarify goals of care, address barriers to treatment adherence, and promote evidence‑based therapy interventions. Drawing on your understanding of both clinical practice and health insurance processes, you will review requests for services, help coordinate authorizations, and support efficient, high‑quality delivery of home‑based physical therapy and related services. You will identify members at risk for functional decline or avoidable hospitalization and proactively engage them and their care teams with education, coaching, and referrals to appropriate community and rehabilitative resources. As a key member of a collaborative, patient‑centered organization focused on community health improvement, you will help strengthen provider relationships in the Franklin area, maintain an effective network of home health and rehabilitation partners, and serve as a clinical resource on mobility, falls prevention, and functional restoration. This role requires strong communication, documentation, and organizational skills, comfort working within electronic medical record and care management systems, and the ability to work independently in the field while staying closely connected to the broader CarePartners team. The ideal candidate is a licensed physical therapist with experience in home health, outpatient, or acute care settings who is passionate about coordinated care, has strong clinical assessment skills, and is eager to contribute to a compassionate, collaborative culture that puts patients and communities first.
