Physical Therapist, PRN

Description Do you have experience working within a Home Health Agency or working in a professional healthcare facility? JFSA Caring at Home has an immediate opening for a PRN Physical Therapist to provide comprehensive, professional, and compassionate PT services to clients in their homes. The Physical Therapist assesses and treats clients according to the individualized plan of care; monitors and documents the client's condition, progress, and outcomes; and provides client and caregiver. Services are provided under a physician's order and in accordance with home health agency regulations, federal and state laws, and agency policies. Duties & Responsibilities:
- Provides PT assessments, diagnostic tests, and skilled treatments, and ongoing evaluations of clients who are receiving services under a medically approved Plan of Care.
- Participates in the development of the Plan of Care.
- Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
- Prepares a report of each client's diagnostic evaluation and makes recommendations.
- Prepares clinical and progress notes.
- Instructs clients and their families in Activities of Daily Living.
- Maintains confidentiality in all aspects of the job.
- Attends care conferences as deemed necessary.
- Participates in in-service programs as requested.
- Performs other duties as assigned.
- Applicants must be licensed in the state of Ohio and be a graduate of a recognized and accredited occupational therapy program, per home health agency regulation.
- Home health agency or experience in a health care facility is required.
- Current PT license as required by state(s) of practice.
- CPR certification.
- Current Ohio Driver's license, reliable transportation, and current automobile insurance.
