Position Overview
As an Intensive Case Manager, you will serve as a primary support and advocate for individuals receiving behavioral health services. You will coordinate care, connect participants to community resources, develop individualized service plans, and ensure continuity of care that promotes recovery, wellness, and community integration.
This role combines assessment, care coordination, crisis support, advocacy, and rehabilitation planning to help individuals successfully live, learn, work, and socialize within their communities.
Key Responsibilities
Care Coordination & Service Planning
- Coordinate and monitor a comprehensive continuum of services to meet participant needs.
- Ensure timely access to behavioral health, healthcare, housing, vocational, educational, and community resources.
- Develop, implement, and update individualized rehabilitation plans focused on participant strengths, goals, and recovery objectives.
- Complete functional assessments and collaborate with participants to identify priorities and support needs.
Participant Advocacy & Support
- Build supportive relationships that promote participant engagement, empowerment, and self-determination.
- Advocate for participants to ensure access to appropriate services and supports.
- Monitor participant progress and assist with navigating complex service systems.
- Identify service gaps and recommend creative solutions that improve outcomes and continuity of care.
Crisis Intervention & Problem Solving
- Participate in the assessment and resolution of crisis situations.
- Identify key support systems, resources, and intervention strategies to promote participant safety and stability.
- Collaborate with treatment teams and community providers to ensure coordinated responses during emergencies.
Documentation & Compliance
- Maintain accurate, timely, and comprehensive documentation in accordance with agency, contractual, and regulatory requirements.
- Document participant contacts, progress, service utilization, transfers, and outcomes.
- Meet established productivity and service delivery standards.
Qualifications
Required
- Bachelor's Degree in Human Services, Social Services, Healthcare, Psychology, Sociology, or a related field.
- Minimum of three (3) years of direct mental health experience.
OR
- An equivalent combination of education, training, and experience that provides the required knowledge and skills.
- Strong verbal and written communication skills.
- Excellent organizational skills and attention to detail.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Proficiency in Microsoft Outlook, Word, internet-based systems, and electronic documentation.
- Ability to meet direct service and documentation requirements.
- Valid driver's license with an acceptable driving record.
Preferred
- Experience using electronic health record (EHR) systems.
- Advanced Drug & Alcohol Counseling Certification.
- Knowledge of community behavioral health resources and recovery-oriented services.

Work Environment
This position may require:
- Frequent communication, active listening, and participant engagement
- Local travel within the community
- Physical activities including lifting, carrying, kneeling, bending, reaching, and assisting participants when necessary
- Crisis intervention and emergency response
- On-call participation, including carrying a communication device during assigned shifts
- Reporting to work during emergencies, including severe weather conditions
- Meeting established productivity and documentation expectations
