Pay Rate : Min $63,793.60 Mid $79,747.20Position Summary
Provides a one year postgraduate training program to pharmacists in order to achieve professional competence in the delivery of patient-centered care and pharmacy services. The PGY2 program's purpose is to build on the Doctor of Pharmacy education and PGY1 postgraduate training to contribute to the development of clinical pharmacy specialists responsible for advanced medication-related care of patients within a particular patient population. The resident reports to the PGY2 Residency Director and the Director of Pharmacy, and will work within the scope of the Pharmacy Practice Act. The resident assesses, plans, recommends, implements, and monitors pharmaceutical care according to LRH policies and procedures. The resident will use independent judgment in developing treatment recommendations for specialty populations, and will closely monitor and adjust treatment recommendations as needed. The resident is responsible to practice within his/her scope of licensure in accordance with the LRH standards of clinical pharmacy practice. The resident performs other duties as assigned and is provided guidance by the residency director, preceptors, and the department leadership team. The resident is also required to identify opportunities for and contribute to the improvement of quality, safety, cost-containment, as well as patient, customer, and employee satisfaction. - Must meet all requirements of a pharmacist required by LRH and additional requirements as outlined in the LRH Residency Manual as endorsed by the Residency Advisory Council. Meets all requirements as outlined in the PGY2 ASHP Accreditation Standard. Remains current with all LRH mandatory annual education.
- Perform complex pharmaceutical care functions for patients within assigned area of specialty. Completes all documentation in the patient chart and monitoring tools for subsequent pharmacists, when applicable.
- Collaborate and coordinate with physicians, patient care teams, and focused professionals to ensure positive patient outcomes through cost-effective, efficient medication and resource utilization.
- Demonstrate knowledge of all equipment and systems/technology necessary to complete duties and responsibilities. Successfully perform service commitment (staffing) as determined by departmental need directed by the pharmacy leadership team in collaboration with the Residency Advisory Council.
- Demonstrate responsibility to the patient by assessing and developing a complete and detailed treatment plan for each individual being cared for by the team.
- Demonstrate ability to manage formulary and medication-use processes, as applicable to the organization. Evaluate and investigate practice, review data, and assimilate scientific evidence to improve patient care and/or the medication use system.
- Fulfill role of a medication expert by continuously seeking to learn more about the unique pharmaceutical needs of specific patient populations and integrating this information into the performance of all other primary duties.
- Be a role model through professional development, continuous learning, and improvement of competence in the practice of pharmacy and area of specialty. Coach, teach, and mentor others to continuously increase competence of team.
- Promote patient, physician, staff, and guest satisfaction by exhibiting LRH cultural behaviors..
- Serve as a resource within the area of specialty to all staff members (physicians, pharmacists, nurses, and all members of patient care teams).
