Responsibilities A day in the life of a Licensed Practical Nurse at Hackensack Meridian Health includes: Supervises and assigns CNAs on the unit. Follows the nursing care plan and utilizes nursing diagnoses in cooperation with the supervisor and unit personnel. Follows the plan of care for residents, consistently, accurately, and independently prioritizing and completing routine assignments within guidelines and schedules. Recognizes psychosocial needs, their possible effects on behavior, and intervenes to provide emotional support. Maintains accurate records of all patients in the unit. Identifies and reports all real or potential problems regarding the development or administration of patient care. Records, reports, and interprets patient responses to treatment and care to appropriate members of the inter‑disciplinary team. Identifies changes in patient conditions/needs and reports to charge nurse. Demonstrates a team approach to problem solving through frequent communication and collaboration on complex issues to provide appropriate care. Maintains all equipment as per facility policy and uses equipment consistent with specifications. Participates and represents Harborage and Nursing staff on committees as requested by supervisors. Identifies patient needs and treatment opportunities, providing services to meet those needs on an ongoing basis. Maintains professional competence and stays current with occupational health trends through continuing education, in‑service programs, professional staff sessions, and training of nursing personnel. Fosters good relationships and communications with all team members, patients, families, and visitors. Assists with new team member orientation and provides key experiences to prepare teammates for independent performance of essential job functions. Reports problems to Manager promptly. Other duties and/or projects as assigned. Adheres to HMH organizational competencies and standards of behavior. Qualifications Required Completion of an approved LPN program. Current with all continuing education (CE) requirements. Perform clinical care professionally. Serve as patient advocate and role model to other team members. Deliver patient care with minimal disruptions and obstacles. Ability to effectively work on a multidisciplinary team. Excellent written and verbal communication skills. Proficient computer skills, including Microsoft Office and/or Google Suite platforms. Endurance to stand and walk for extended periods of time. Ability to lift, push, and pull a minimum of 50 pounds. Preferred Long‑term care facility experience. Prior work experience with geriatric and disabled populations. Bi‑lingual (Spanish). Licenses and Certifications Required Licensed Practical Nurse. BLS AHA Basic Health Care Life Support. HCP Certification within 60 days of entering position. Preferred IV Certification. Compensation Minimum rate of $32.97 hourly.
