Case Manager 1

Overview The Case Manager 1 is an entry level position responsible for providing case management with individuals and their families to establish appropriate linkages, referrals, coordination of care, and to provide regular follow‑up on needed services and supports under the supervision of a qualified case management supervisor. The Case Manager 1 will identify and assess client needs and evaluate, coordinate, and ensure necessary services are provided. Required Education and Experience Bachelor’s degree from an accredited college or university in an applicable human services field (including social work, counseling, behavioral science, psychology, or a related field).
