Chatham Glen Healthcare & Rehab Center, located in the heart of The Villages, FL, is currently seeking a dedicated Physical Therapist (PT) to join our in-house Subacute Rehabilitation team. We proudly deliver high-quality, patient-focused care through a fully integrated therapy model — no contract providers.
Whether you're looking for full-time, part-time, or per diem work, we offer flexibility, professional support, and a rewarding clinical environment.
Key Responsibilities:
- Evaluate and treat patients with physical impairments, functional limitations, and disabilities
- Develop individualized treatment plans with measurable, outcome-focused goals
- Collaborate with an interdisciplinary team including occupational therapy, speech therapy, nursing, and medical staff
- Document all services provided, track patient progress, and adjust treatment plans as needed
- Educate patients, families, and caregivers on home exercise programs and rehabilitation goals
Qualifications:
- Licensed Physical Therapist in Florida (or eligible for licensure)
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Prior experience in a skilled nursing or subacute rehab setting is a plus
- Strong communication, clinical reasoning, and time management skills
