Compassionate Hospice RN Case Manager

St. Croix Hospice is seeking an experienced RN Case Manager to lead each patient’s hospice care team and manage the Plan of Care in collaboration with the Hospice Medical Director, patient, and family. The role includes direct patient care, medication administration, and coordinating with physicians and the care team to improve quality of life. The ideal candidate holds an active RN license, can navigate HIPAA requirements, and is available for on-call shifts including evenings and weekends. #J-18808-Ljbffr
