Home Health AideNeuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support — partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential.We're looking for a compassionate Home Health Aide to provide hands-on personal care and support to patients in their own homes. This is an on-site role — you'll be working directly with patients and families every shift, following the care plan set by the supervising nurse.Assist patients with activities of daily living, including bathing, dressing, and grooming.Prepare meals and assist with feeding as needed.Take and record vital signs, and report changes in patient condition to the supervising nurse.Perform light housekeeping related to the patient's care.Assist with mobility, transfers, and safe positioning.Provide companionship and emotional support to patients and families.Follow the individualized care plan established by the RN and care team.Complete required visit documentation accurately and on time.Current NJ Home Health Aide (HHA) or CHHA certification.CPR certification and up-to-date health screenings.Reliable transportation to reach patients' homes.Patience, compassion, and strong communication skills.
