Responsibilities

• Provide direct patient care, including administration of medications, wound care, and vital sign monitoring, under the supervision of a registered nurse.

• Develop and implement individualized patient care plans in collaboration with interdisciplinary teams.

• Participate in shift reports, patient assessments, and quality improvement initiatives to ensure optimal patient outcomes.

• Utilize effective communication skills to collaborate with patients, families, and healthcare professionals to address patient needs and concerns.

• Maintain accurate and timely records, including patient charts and medication records.

Requirements

• Current LVN licensure in the state of Texas.

• High school diploma or equivalent required.
