Licensed Practical Nurse/ Vocational Nurse- Geriatrics and Extended Care

Licensed Practical NurseThe LPN is a key member of CLC they will contribute to the plan of care by collaborating with interdisciplinary team members, the patient and patient's family. Duties include but are not limited to:Demonstrates ability to accurately report and record significant patient information and uses good judgment in selecting the most appropriate application of procedures and guidelines, varying the order and sequence of assigned procedures based on the patient's condition.Collaborate with other healthcare providers including RN's, physicians, therapists and social workers, to develop and implement care plans tailored to each resident's needs.Provides teaching activities of daily living (ADL's) to patients and encourages them to participate in ADL's leading to increased independence.Provides or reinforces patient teaching related to medications, disease process, procedures, and self-care activities. Participates in more complex teaching for patient self-care, such as diabetic care, self-catheterization, etc.Supports a home-like environment in which Residents are offered choices and preferences are honored about activities of daily living including wake, sleep, and mealtimes, bath schedules, recreational activities, etc.Manages wounds, pain, clinical conditions, and provides emotional support to the CLC Residents and their families as needed.Knowledge and skill to sufficiently prepare, administer, and document actions specific to medication administration including oral, sublingual, subcutaneous, intravascular, and intramuscular routes.LPNs must possess strong clinical and astute observational skills, experience in geriatric population care, pain management experience, and the ability to provide extensive emotional support.Support family members with care and compassion during- the end-of-life process and assist with the necessary arrangements for the body after death.Performs other duties as assigned.Work Schedule: 12 hours nights and day shifts 7:30am - 8:00pm and 7:30pm- 8:00am with rotating weekends and holidaysRecruitment/Relocation Incentives (Sign-on Bonus): Authorized $5,000Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Telework: Not eligibleVirtual: This is not a virtual position.Functional Statement #: 51243F, 55273F, 55274F, 55275FPermanent Change of Station (PCS): Not AuthorizedNotifications:This position is in the Excepted Service.The salary is based on the grade and step approved for the selected candidate.Veterans' preference does not apply for internal or other current permanent Federal agency employees.The 2-page Resume requirement does not apply to this position.
