We offer great benefits including: Responsibilities As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Clark Manor by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals. Perform resident assessments and assist in the discharge process. Ensure residents are comfortable and safe through knowledge, skills, and attention to detail. Qualifications Unencumbered State of Illinois RN License. Graduate of an accredited nursing program. CPR Certification. Prior experience as a MDS Coordinator is preferred. One (1) year of experience as an RN in a Skilled Nursing Facility setting. One (1) year of experience as a CNA is preferable. We are willing to invest in the right candidate. Benefits Daily pay.
