Hospice Registered Nurse Case Manager
Community Hospice of Northeast Florida
Community Hospice is seeking an RN Case Manager to plan and manage nursing care for an assigned caseload in home or long-term care settings. You will collaborate with the interdisciplinary team to achieve patient- and family-directed goals using a holistic approach. Responsibilities include education, coordinating care across sites, and documenting thoroughly. Flexibility to work across care settings is required, and you will support patients, families, and staff while adhering to hospice #J-18808-Ljbffr
