Hospice RN Case Manager , Care
Gentiva
Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate in-home care and coordinate with an interdisciplinary team across various settings. You will guide patients and families through the hospice journey, document care plans, and ensure quality outcomes.
The role requires an active RN license, at least one year of RN experience in hospice, home health, or acute care, and strong communication skills to educate and support families during a challenging time.
