Responsibilities:

• Provide comprehensive nursing care to patients in their home setting.
• Conduct OASIS assessments and documentation accurately using HCHB EMR.
• Develop and implement individualized care plans based on patient needs.
• Educate patients and their families on disease management and health maintenance.
• Collaborate with interdisciplinary teams to ensure quality patient care.
• Ensure compliance with all healthcare regulations and standards.
