Pride Health is hiring a Physical Therapist (PT) to support our client's healthcare facility in Granite City, IL.

Float / Coverage Area: Madison & St. Clair Counties, IL

Job Type: Local contract

Contract Length: 14 Weeks Pay Range: $50.00 to $57.00/hour*

Mileage Reimbursement Rate (if applicable): $0.51/ mile *Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Conduct home health physical therapy admissions and evaluations.
• Perform discharge visits and assess patient progress toward established goals.
• Develop and implement individualized physical therapy plans of care.
• Provide skilled PT interventions in the patient's home.
• Monitor patient progress and modify the plan of care as needed.
• Supervise and provide clinical direction to Physical Therapist Assistants (PTAs).
• Communicate effectively with patients, families, physicians, and the interdisciplinary care team.
• Complete required clinical documentation accurately and timely.
• Maintain compliance with home health standards, policies, and regulations.

Requirements:

• Current Illinois Physical Therapist (PT) license required.
• Illinois is not a compact state, so an active Illinois license is required.
• Home health experience is preferred.
