Responsibilities:

Provide direct patient care including monitoring vital signs, administering medications, and wound care

Assist in developing and implementing individualized patient care plans

Collaborate with RNs, physicians, and interdisciplinary care teams to ensure quality outcomes

Document patient assessments, treatments, and progress accurately in medical records

Educate patients and families on health management, medications, and discharge instructions

Respond promptly to patient needs and changes in condition, escalating concerns as appropriate

Maintain compliance with all state, federal, and organizational healthcare regulations

Requirements:

Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania

Graduate of an accredited LPN/LVN nursing program

Current CPR/BLS certification

Strong clinical assessment and critical thinking skills

Excellent communication and interpersonal skills with patients and care teams

Ability to work independently and as part of a multidisciplinary team

Compassionate, patient-focused demeanor with a commitment to quality care
