Locations Self Regional Healthcare Greenwood, SC 29646, USA Description Provides Physical Therapy (PT) services to patients by means of assessment, planning and implementation of treatment. Remains proficient in implementation of clinical procedures such as therapeutic activities, modalities, adaptations, and patient and family education and assessment of durable medical equipment (DME) needs and uses these skills in the home setting. Maintains personal professional growth through continuing education, independent study, shared information, and training with team members. Fosters open communication and rapport with other disciplines, physicians, all hospital disciplines and outside agencies as needed to ensure patient needs are met. Completes all documentation per Home Health policies. Performs all other duties as assigned by Home Health Services (HHS) Managers or Directors. Graduate of a Physical Therapy program. Must be licensed as a Physical Therapist in SC. Home Health experience preferred. BLS certified. Qualifications Education — Bachelors or better in Pre-Physical Therapy. Licenses & Certifications — Required : PT Experience — Home Health experience preferred. Responsibilities Provides PT services in the home health setting, including assessment, planning, and treatment implementation.
