Description

• Conduct evaluations of patients in both inpatient and outpatient environments, creating personalized treatment plans tailored to their needs
• Deliver physical therapy interventions that focus on optimizing mobility, strength, balance, and overall function
• Partner with nurses, doctors, case managers, and other rehab specialists to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge transitions, and home exercise regimens

• Engage in care team meetings and interdisciplinary conferences to ensure seamless patient care

Education

• Doctor of Physical Therapy (DPT) or similar degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
