- Conduct evaluations of patients in both inpatient and outpatient environments, creating personalized treatment plans tailored to their needs
- Deliver physical therapy interventions that focus on optimizing mobility, strength, balance, and overall function
- Partner with nurses, doctors, case managers, and other rehab specialists to maximize patient outcomes
- Guide patients and their families through therapeutic objectives, discharge transitions, and home exercise regimens
- Document assessments, treatments, and patient progress in accordance with facility and regulatory requirements
- Engage in care team meetings and interdisciplinary conferences to ensure seamless patient care
- Doctor of Physical Therapy (DPT) or similar degree from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification preferred
- Specialty certifications (e.g., neurological, geriatric) are beneficial but not mandatory
- Prior experience is highly preferred
- Ability to support patients with multifaceted medical and mobility challenges
- Excellent communication and collaboration abilities
- Familiarity with digital documentation and clinical charting systems
