Overview The Senior Integrated Health Care Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health and dental services. The Senior Integrated Health Care Clinician will foster positive working relationships with co-workers, clients and external key stakeholders. Location: Plainville, CT. Salary and Benefits Salary: $68,000.00 Sign-on bonus: $10,000 (half paid at start, half paid at 1 year of service). Loan repayment eligibility: Eligible for up to $250,000 in loan repayment through the National Health Service Corps (NHSC). Education and Experience / Qualifications Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field, or a doctoral degree in Psychology is required. Licensure requirements for clinical positions: Must be fully licensed in CT (LPC, LMFT, LCSW, Licensed Psychologist). Provides behavioral health consultation to the primary care provider, assists clients in meeting medical treatment compliance, and refers clients to Wheeler services and/or community services as needed. Conducts program-specific screening and evaluation and formulates diagnosis as indicated by DSM guidelines. Develops and documents clear clinical formulations and treatment plans that are individualized, measurable, and developed in collaboration with the client/family and others as relevant. Demonstrates knowledge of psychopathology and primary modalities of psychotherapy (individual, group, family). Correctly triages clinical emergencies (including suicide and danger to others). Uses best practices and evidence-based models where appropriate. Facilitates focused, time-limited group therapy as needed. Provides or refers to group therapy when required. Demonstrates knowledge of psychotropic medications. Completes clinical documentation in a timely manner. Facilitates group therapy (outpatient or intensive outpatient) and/or sees patients across the lifespan when necessary. Knowledge of internal and external levels of care and referral processes. Makes clinically appropriate referrals in a timely manner. Communicates effectively with clients, families, and internal/external providers. Attends required meetings and multidisciplinary treatment planning meetings. Meets direct service productivity goals as defined by the program. Adheres to program/department/agency goals, data collection, and reporting protocols. Documents all client-related information in compliance with agency policy. Attends supervision and seeks additional supervision or consultation as needed. Collaborates with community providers to support positive system development and Wheeler Clinic’s reputation. Provides culturally competent care and respects clients’ cultural traditions, values, and beliefs. Reports child abuse/neglect as mandated and adheres to confidentiality guidelines. Ensures compliance with licensing, regulatory, and accreditation standards related to the program, staff, records, facility, and care of youth.
