- Conduct assessments of patients in both inpatient and outpatient environments and create customized treatment plans.
- Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
- Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes.
- Guide patients and their families through therapeutic goals, discharge processes, and home exercise routines.
- Maintain accurate documentation of evaluations, treatments, and patient progress in line with regulatory and facility standards.
- Engage in multidisciplinary team rounds and collaborative conferences to ensure cohesive patient care.
- Doctorate in Physical Therapy (DPT) or an equivalent accreditation from an accredited institution.
- Possession of state licensure as a Physical Therapist
- CPR certification highly desirable.
- Specialty certifications (e.g., in neurological or geriatric care) are beneficial but not mandatory.
- Prior experience is strongly preferred.
- Ability to assess and treat patients with complex medical histories and mobility challenges.
- Excellent communication and collaboration skills.
- Comfortable working with digital charting systems and clinical documentation.
