Discharge & Care Transitions Navigator
Methodist Dallas Medical Center
Methodist Charlton Medical Center in the Dallas area is seeking a Care Transitions Navigator to coordinate activities that promote quality outcomes, patient throughput, and discharge planning. The role will identify barriers to throughput and minimize delays in discharge plans while supporting appropriate resource utilization. The position requires a Social Work degree (MSW preferred) or RN with BSN, relevant hospital case management experience, and the ability to manage multiple priorities in a #J-18808-Ljbffr Methodist Health System
