Registered Nurse - Case Manager

Join our team as an RN-Case Manager, providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses caused by exposure to toxic substances. Key Responsibilities Include Conduct weekly in-home nursing visits, including physical assessments, medication management, home safety checks, and overseeing caregivers. Develop and evaluate care plans, coordinate with clients and healthcare teams, and ensure quality of care. Serve as a clinical resource on-site, support staffing and scheduling, and adhere to agency policies and scope of practice. Participate in staff training, review clinical documentation, and contribute to performance improvement initiatives. Assist with client and physician engagement and perform supervisory visits as needed. Qualifications Include Graduate of an approved nursing program with an unrestricted RN license. BSN preferred, with at least two years of nursing experience, including one in home care. Current CPR certification and supervisory or case management experience preferred.
