Home Health/Hospice Registered Nurse (RN) - San Mateo County

Positions and Compensation Job Pay: $50 - $65 an hour, $100 - $200 per visit. Location San Mateo County Palo Alto Foster City Job Description The Registered Nurse is a professional nurse who coordinates and directs the home care patient’s care based on individual patient needs. The RN is responsible for independent management of the Home Health/Hospice patient population using advanced assessment, teaching, and decision‑making skills. Responsibilities include ensuring appropriate referrals, regular interdisciplinary conferencing, and complete documentation. The RN integrates new patient populations and applies relevant knowledge consistently. Responsibilities Assessment Assesses physical, functional, psychosocial, and cognitive status of the home care patient using interview, observation, and physical exam techniques. Assesses the home environment for safety, infection control, and community resource needs. Collects information for assessment with the patient, family, physician, and health care team members. Incorporates multidisciplinary data into the nursing assessment of the home care patient. Provides ongoing nursing assessment to determine physiological or psychosocial risk. Assesses for the presence of advanced directives and facilitates further action if indicated. Planning Plans with the patient, family, and physician for care feasible within physical, financial, and emotional resources. Establishes individualized, measurable goals in consultation with the patient, family, and other health care providers. Anticipates home care needs and seeks community resources to facilitate problem solving. Develops standards of care for patients in the home setting. Utilizes home care standards and regulatory guidelines in developing an individualized care plan. Maintains working knowledge of community resources and refers patients and families appropriately. Implementation Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments for varied patient populations. Maintains technical skills according to agency standards, as measured by competency assessments during orientation and annually. Implements safe, competent care with home care patients and families within the Home Health Agency’s framework. Documents all clinical care and updates the primary physician as required, meeting deadlines for certification, re‑certification, and billing documentation. Provides patient, family, and caregiver teaching and instructions per assessment and plan of care. Coordinates discharge planning and acts as Case Manager when assigned. Participates in on‑call duties per policy and ensures equipment and services are available. Instructs, supervises, and evaluates home health aide care every two weeks. Requirements Current RN License in the State of California. Minimum 1 year of professional nursing experience within the last three years in public health, home care, or hospice settings. Current CPR certification. Licensed driver with a car insured per state or organization requirements. Compensation Details Hourly wage: $50 - $70. #J-18808-Ljbffr
