An Operating Room Registered Nurse (OR RN) , also known as a perioperative nurse, provides high-quality patient care across all phases of the surgical experience: preoperative (before surgery), intraoperative (during surgery), and postoperative (immediately following surgery). Acting as a critical patient advocate, the OR RN coordinates closely with surgeons, anesthesiologists, and surgical technologists to ensure safe, sterile, and efficient operative environments.


Job Summary
The OR RN delivers technical and professional nursing care within the surgical suite. This role requires rapid critical assessment, deep knowledge of aseptic techniques, and the ability to maintain composure under high-stress conditions. Depending on assignment, the OR RN may alternate between the roles of a Circulating Nurse (managing the overall room environment) and a Scrub Nurse (working directly within the sterile field).


Key Responsibilities

1. Preoperative Care (Before Surgery)

• Patient Preparation: Review medical histories, allergies, lab findings, and signed surgical consent forms.
• Safety Protocols: Implement surgical safety checklists to verify patient identity, surgical procedure, and correct operation sites.
• Room Setup: Ensure all necessary surgical instruments, medications, and equipment are sterile, functioning, and properly prepared for the specific procedure.
• Patient Advocacy: Address patient anxiety, answer final questions, and provide emotional support.

2. Intraoperative Care (During Surgery)

• Circulating Duties:
  • Monitor and manage the unsterile environment of the operating suite.
  • Anticipate surgical team needs and manage the flow of equipment and supplies.
  • Track and document surgical events, fluid outputs, and instrument/sponge counts to prevent retained objects.
• Scrub Duties:
  • Maintain strict adherence to aseptic and sterile techniques.
  • Hand sterile instruments, sponges, and solutions directly to the surgeon during the procedure.
• Patient Monitoring: Collaborate with the anesthesia team to continuously observe patient vital signs and physical positioning on the table.

3. Postoperative Care (After Surgery)

• Handoff Reporting: Safely transfer patients to the Post-Anesthesia Care Unit (PACU) and deliver clear, detailed reports to recovery staff regarding surgical events and potential risks.
• Post-Op Documentation: Finalize patient charts, electronic medical records, and inventory logs.
• Sterilization Oversight: Ensure used instruments and materials are accounted for and routed properly to central sterilization per protocol.
