Hospice Admissions RN — On-Call Care Navigator

Affinity Health Management

Affinity Health Management is seeking an Admissions Nurse to join our hospice team in Cincinnati. You will perform skilled nursing assessments, care planning, and on-call support to maximize patient comfort and align with hospice policies. Working with the interdisciplinary team, you will coordinate admission care, educate patients and families, and ensure accurate documentation and communication with physicians and case managers. Ohio RN license and hospice experience are required. #J-18808-Ljbffr Affinity Health Management
