$1217.88/week - Job Summary: Provide licensed practical nursing care in a skilled nursing facility, ensuring patient comfort, safety, and well-being. Administer medications, monitor patient conditions, and collaborate with healthcare teams to support recovery and health maintenance. Responsibilities: Perform comprehensive patient assessments and document clinical observations. Administer medications and treatments according to physician orders and facility protocols. Monitor patient vital signs and respond promptly to any changes in condition. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Educate patients and their families on care procedures and health management. Qualifications: Hold an active LVN or LPN license valid in the state of practice. Have recent experience working in a LTC or SNF setting. Demonstrate excellent patient care and communication skills. Be able to multitask efficiently in a fast-paced environment. Complete a criminal background check and provide up-to-date immunization records, including COVID-19 vaccination or approved exemption.
