Certifications:

• Associate Degree in nursing required. Bachelors of Science in Nursing (BSN) preferred.
• Current California Registered Nurse license required.
• Current BLS/Healthcare Provider status as per American Heart Association standards required.
• BLS - AHA
• California Registered Nurse License

Mandatory Experience:

• Transitional Care Coordinator
• 3 years’ nursing experience required.
• Ability to demonstrate a working knowledge of community resources, post-acute care coordination, and case management principles required.
• Bilingual in Spanish preferred.
• Case Management experience preferred.
• Broad general knowledge of nursing and possess the ability to effectively navigate and utilize a computerized medical record system. 
• Epic experience needed ( recent)
• Case Management experience desired
• Shift: Monday – Friday-13 weeks 7:30a – 4p
