Job Title Transition Facilitator Division/Program In Home Outreach Team - Alameda Starting Compensation $22.36 – $26.83 USD Per Hour Working Location San Leandro: San Leandro Center (In-person position) Working Hours / Shift Full Time, Monday‑Friday, 8:00am‑5:30pm Why Join Our Team? What You Bring (Qualifications) Education: High School Diploma or equivalent; enrollment in an Associate’s Program strongly preferred. Experience: Personal experience living with a mental‑health diagnosis and/or recovery from substance use disorders, or having supported someone with a mental‑health diagnosis or substance use disorder. Licensing: Valid California Driver’s License with current automobile insurance; must maintain a driving record that meets the company's driver eligibility policy. How You Will Make a Difference (Job Overview) This position has primary responsibility for using your lived experience and knowledge of the system to provide outreach, engagement, support, and advocacy to Transition Age Youth (TAY) and their families who need mental health services. Division / Program Overview The IHOT team provides outreach and engagement services to disengaged TAY (ages 18‑25) throughout Alameda County. The team is comprised of peer advocates who use their lived experience to help youth connect with services. The team lead provides clinical oversight.
