Social Service Assistant

The Social Service Assistant, under the supervision of the Director of Social Services, coordinates the resident admissions to DeKalb County Rehab & Nursing Center.

• Orients new admissions to facility.
• Responds to initial questions and conducts facility tours.
• Completes social service assessments, social history, discharge plans, and resident profiles.
• Coordinates new residents' integration into the therapeutic and social milieu of the facility.

The Social Service Assistant is accountable for assisting the resident / family with adjustment to the facility and continual integration of the medical, social, psychological and spiritual aspects of the residents' care through-out their stay at the nursing home. Requires ability to establish and maintain satisfactory working relationship with facility staff, residents and their families and the general public.

This full-time position will work 38.75 hours per week, 9:00 a.m. to 5:00 p.m., with rotating weekend on call and the possibility of occasional evening work.
