About the Facility – Veterans Home of California - Fresno

Opened in October 2013, the VHC-Fresno has 300 beds and presently offers Residential Care Facility for the Elderly (RCFE), also known as assisted living. As a new service, the Home is offering Skilled Nursing. The two levels of care provide continuity in the lives of residents in a homelike atmosphere of dignity and respect. All healthcare needs, meals, transportation and activities are provided by the Veterans Home of California Fresno. Skilled nursing care is provided to residents on a 24-hour basis by licensed nurses and certified nursing assistants. Skilled nursing residents have greater access to rehabilitation therapies, nursing care, pharmacy management, structured activities, and clinical dietary services. A memory care program with this level of care provides a supervised environment for veterans with symptoms of confusion, memory loss, difficulty making decisions, solving problems or participating in conversations. The Veterans Home provides California veterans with a living environment that protects their dignity and contributes to their feeling of self-reliance and self-worth. Responsibilities

• Organize, provide, and maintain health care services consistent with CalVet policies and procedures.
• Plan and implement nursing care. This includes administration or medication, therapeutic agents, treatments, disease prevention, physician's orders, as well as obtaining specimens for diagnostic testing.
• Participate as a member of the Inter-disciplinary team in developing multi-disciplinary treatment efforts.
• Complete assigned summaries and formulate care plans, assist physicians with rounds note and institute physician orders.
• Collect information regarding patient status and report the results to the Primary Care Provider (PCP) or RN in a timely manner.
• Provide Resident education and accurately document care in the approved format.
• Assist mental health professionals by contributing to the treatment planning processes and the formal evaluation of Resident progress by monitoring and recording observed patient behavior.
• Document in the Resident's health record in the nursing care and treatment provided and the Resident's response to facilitate the continuity of care and communicate to other health care providers.
• Ensure that staff utilizes optimum standards of resident care and that staff members observe safety and Infection Control Precautions and regulations at all times.
• Be aware of residents' rights, identifying the potential or resident abuse, report promptly to physicians or nursing supervisor about any adverse behavior, resident abuse or suspected abuse by staff.
• Float to other units as necessary. Answer call-lights promptly.
• All nursing care and treatment plans shall be recorded properly on the Medication Administration Record (MAR), and Treatment Administration Record (TAR).
• Assume responsibility for completing consents, laboratory reports, and charts.
• Make rounds as requested and immediately report unusual resident conditions to physician or the charge nurse. Must adhere to the policies and procedures of the facility.

Requirements

• Experience: Shall have a minimum of one year within the last three years.
• Professional license in California.
• BLS from a recognized program (AHA or Red Cross).
• COVID Vaccination: Needs to be fully vaccinated including booster, if eligible, or have a medical or religious exemption*. Medical exemption: no information needed but exemption does need to state a timeframe or if indefinite, needs to state as such. If not fully vaccinated, required to get tested twice a week.
