Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day and Mid Hours per Shift: 8 hours Experience: Minimum of 5 years of experience required License: RN License required Certifications: BLS, CCM or ACM certification preferred Must-Have: - Acute care experience - Direct hospital-based discharge planning experience to the community - Strong inpatient and Emergency Department case management experience - Knowledge of California insurance requirements and county regulations - Ability to function independently in an acute care hospital setting Description: A Case Manager is required to be cross-trained on all units to work as needed. The position requires a seasoned inpatient Case Manager RN. Candidates should possess direct hospital-based discharge planning experience to the community. Experience in inpatient and Emergency Department case management is crucial. Knowledge of California insurance requirements and county regulations is necessary, especially for complex discharges. The role involves variable start times with every other weekend work and floating to various units as needed. Independent functioning in an acute care hospital setting is essential.
